Link Between Heart Health and CDP
When planning for the second five-year phase of the Ontario Heart Health Program-Taking Action for Healthy Living, 2003-2007 stakeholders at both the provincial and local levels began to consider the value of expanding beyond heart health to a chronic disease approach. This was in line with other trends. For example many health departments now have a chronic disease prevention team. Aligning heart health project mandates with this structure meant the potential for closer alignment of initiatives between the heart health projects and local health unit work.
